Project Coordinator
Dixiebloor Neighbourhood Centre, Chronic Disease Prevention Community Initiative requires a Project Coordinator.
Program Description:
This contract position requires a health promotion specialist who will be responsible for coordinating the Chronic Disease Prevention Community Initiative project, a Healthy Communities Fund initiative of the Ministry of Health Promotion. Phase I of the project concludes March 31,2010 and will focus on a series of language-specific community consultations. Phase II is one year in length and is dependent on the successful completion of Phase I. The project is community led and participant driven.
Reports to: Co-ordinator of Services for Seniors/Advisory Committee
Responsibilities:
- Connect/work with community partners and advisory committee
- Phase I: Organize and facilitate a series of language specific community consultations
- Phase II: Supervise a team of volunteers and staff to deliver activities that address chronic disease prevention
- Network with community partners to deliver the project objectives
- Project documentation as required, statistics and reports to advisory committee
- Develop project evaluation tools
- Participate in the evaluation of the project in collaboration with community partners
- Conduct ongoing outreach and public relations activities
- Ensure the delivery of services in a culturally sensitive manner
- Manage administrative duties associated with the program
- Maintain Health and Safety requirements
- Maintain a co-operative working relationship with other staff members, volunteers, placement students, and partner agency representatives and nurture the collaborative work environment
- Ensure the work environment is inclusive and welcoming to all members of the community
- Other duties that support the operational growth of the project
Qualifications:
- Post-secondary education and/or experience in the human services sector.
- 2-4 years of relevant experience in coordinating health promotion initiatives
- Proven success in project management
- Ability to effectively plan, implement and evaluate project goals and activities
- Sensitivity to working collaboratively in a diverse community setting
- Strong computer skills
- Strong organizational skills
- Excellent written and verbal communication skills
- Use of personal car an asset
- 2nd language an asset
- Police check required
This is a part-time position scheduled to work 28 hours per week
Salary::$24.10 per hour
